Knowledge transfer plan

A knowledge transfer plan is a written schedule of what expertise must move from one person to another before a set date, naming each item, who receives it, and how the transfer is confirmed.

A workable plan has four columns and no more: what the knowledge is, who currently holds it, who is taking it on, and the date it must be confirmed by. Anything longer tends to be filled in for completeness rather than acted on.

The hard part is the first column. Listing responsibilities is easy; listing the things only one person knows takes evidence, which is why the strongest plans are built from the actual record of who touched which decision rather than from an interview.

Confirmation matters more than documentation. A line is only closed when the receiving person has answered a question on it correctly, or has done the task once with the outgoing person watching.
